Facet joint interventions (diagnostic IA/MBB, therapeutic IA/MBB, RFA, and cyst aspiration) are covered for chronic (≥3 months) moderate-to-severe axial neck or low back pain that causes functional deficit, after failure of noninvasive conservative care and exclusion of non-facet pathology. Coverage requires documented baseline and post-procedure pain/disability scores, strict procedural prerequisites (e.g., two diagnostic blocks with ≥80% relief before RFA; ≥50% relief/function for repeat treatments), use of CT or fluoroscopic imaging guidance, and adherence to per-region frequency limits (diagnostic ≤4, therapeutic ≤4, RFA ≤2 per rolling 12 months).
