Advance Care Planning (ACP) is a time-based face-to-face (or CMS-compliant telehealth) counseling service provided by physicians or qualified healthcare professionals to discuss a patient’s wishes for future medical care, and may be offered during an Annual Wellness Visit or as a separate Medicare Part B service. Coverage requires documented voluntary participation, assessment of decision-making capacity, and detailed records of the ACP content, participants, mode, and time; brief E/M conversations and audio-only telephone calls do not qualify. There is no numerical limit on ACP reporting, but repeat claims must be supported by documentation of a change in health status or care preferences.
