Hyaluronic Acid Injections for Knee Osteoarthritis

Intra-articular hyaluronic acid (viscosupplementation) for knee osteoarthritis is covered when all documented criteria are met: symptomatic knee OA with functional impairment, radiographic confirmation, failure or contraindication of at least 3 months of conservative therapy, and failure or contraindication to intra-articular glucocorticoids. Repeat series are allowed only if prior clinical improvement occurred, the initial criteria continue to be met, and at least 6 months have elapsed since the prior series; dosing and frequency must follow FDA-approved labeling and certain contraindications (local infection, allergy) preclude coverage.
