Policy Summary
Serum magnesium testing is covered when there is clinical suspicion of hypomagnesemia or hypermagnesemia—identified by compatible symptoms, impaired renal function, recent large magnesium exposure, or underlying conditions/medications known to alter magnesium balance. Tests tied to these clinical indications are considered reasonable and necessary; tests deemed not reasonable and necessary under Medicare policy are not covered.
