Basivertebral nerve (BVN) ablation is covered for skeletally mature patients with chronic low back pain (≥6 months) as the dominant symptom who have failed at least three conservative therapies and have MRI-confirmed Modic Type 1 or 2 changes between L3 and S1, with no other vertebral pathology. Contraindications include skeletal immaturity, active infection, alternate primary spinal pathologies (e.g., stenosis, radiculopathy), metabolic bone disease or spinal tumor, certain comorbid systemic risks, implantable pulse generators without documented precautions, and ongoing uncontrolled addictive medication use; procedural limits are 1–2 vertebrae per session, max 4 vertebrae lifetime, confined to L3–S1, and no lifetime retreatment of the same vertebral body.
