AI-QCT/AI-CPA using CCTA is medically necessary for patients with acute or stable chest pain without known CAD who are eligible for CCTA and whose cardiac evaluation is negative or inconclusive for ACS, when CCTA shows intermediate clinical risk or CAD-RADS 1–3. The test must be performed only after a completed and interpreted CCTA using FDA-cleared/approved software and interpreted by appropriately trained and credentialed providers; it is not covered for screening, post-MI within 30 days, unstable symptoms, surveillance, high-grade stenosis (CAD-RADS 4–5), normal CCTA (CAD-RADS 0), or when CCTA is contraindicated.
