MIGS is covered when a single trabecular aqueous stent is implanted per eye in adults with mild-to-moderate open-angle glaucoma undergoing cataract surgery and who are currently on ocular hypotensive therapy; standalone subconjunctival or trabecular stents are covered for refractory glaucoma with documented prior filtering/cilioablative failure or mean diurnal medicated IOP ≥20 mmHg on maximally tolerated therapy (usually ≥4 topical classes). MIGS is not first-line for mild-moderate glaucoma, concurrent aqueous shunt placement or multiple simultaneous MIGS techniques with phaco are non-covered, and providers must be credentialed with documented device approval and, for XEN45, surgeon experience in trabeculectomy/bleb management is required.
