MRA (with or without gadolinium) is covered for diagnosis, screening and monitoring of arterial, venous and lymphatic disorders (e.g., aneurysms, AVMs, aortic disease/dissection, carotid/ coronary/renal/mesenteric/ peripheral vascular disease, pulmonary embolism, lymphatic evaluations, intracranial pathology, etc.) when reasonable and necessary and/or can replace a more invasive angiogram; routine physical exams and non‑necessary research are excluded. Coverage requires ordering by the treating physician/practitioner and clinical documentation of necessity (signs/symptoms, prior imaging, intent to avoid invasive testing), documentation of prior gadolinium allergy if contrast used, and confirmation that any implanted device is MRI‑conditional per FDA labeling (MRA contraindications mirror MRI contraindications).
