Genetic testing for heritable thoracic aortic disease is covered for germline tests that have completed a MolDX Technical Assessment when the patient has an aortic root or ascending aortic dilation/aneurysm/dissection and at least one qualifying risk feature (age <60, syndromic features of Marfan/Loeys‑Dietz/vascular Ehlers‑Danlos, or relevant first-/second‑degree family history including unexplained early sudden death). Coverage requires that counseling was offered, the test includes ClinGen/ACC‑AHA–recommended gene/variant content that can be reasonably detected, single‑variant testing is limited to known familial variants, and testing must not conflict with repeat germline testing provisions in policy L38274.
