Superficial Radiation Therapy (SRT) for the Treatment of Nonmelanoma Skin Cancers (NMSC)

SRT is covered for cutaneous low- or high-risk BCC, low- or high-risk SCC, and SCC in situ when the patient is documented as a nonsurgical candidate and risk stratification follows NCCN/ASTRO/AAD guidelines. SRT is excluded for very-high-risk SCC, lesions >4 cm or >6 mm depth, aggressive morphology, perineural/perivascular invasion as sole therapy, previously irradiated/overlapping fields, contraindications to RT, and as first-line therapy in surgical candidates; EBT and HRUS-guided SRT are not covered. Medical records must document clinical measurements, guideline-based risk stratification, nonsurgical candidacy, and that qualified providers and compliant facilities performed the service.
