Injection of tendon sheaths, ligaments, bursae, and ganglion cysts is covered to relieve substantial pain or significant functional disability caused by inflammation or pathology when conservative therapy has failed, is contraindicated, or is inappropriate, and there is reasonable likelihood of meaningful improvement. Services require documentation of detailed pain history, prior treatments, physical exam, diagnostic testing, and appropriate provider credentialing; all injections at a single anatomical site on the same date are considered one injection service, and general anesthesia is contraindicated for diagnostic blocks.
