Covers assessment and treatment of sexual dysfunction in males and females — including comprehensive history/physical, laboratory testing, pharmacologic erectile testing (intracavernosal injection), nocturnal penile tumescence, vascular and cavernosal studies (e.g., Doppler with ICI, cavernosography/cavernosometry), and therapies such as intracavernosal agents, MUSE, oral medications, vacuum devices, penile implants, venous/vascular surgery, and Peyronie’s treatments. Coverage is generally limited to documented organic or structural causes (e.g., diabetes, vascular disease, penile trauma, spinal cord injury, Peyronie’s); many diagnostic tests/procedures (e.g., CCEMG, SPACE, dorsal nerve conduction, plethysmography) are excluded as experimental, penile revascularization has strict age, injury and health criteria, and contractual exclusions (including gender‑reassignment services and psychogenic dysfunction limits) apply.
