Vestibular function testing — including ENG/VNG, caloric testing, and rotational-chair assessments (with VEMP noted as being evaluated) — is covered to evaluate patients with dizziness, vertigo, imbalance, or other signs of vestibular dysfunction when clinical evaluation, including positional maneuvers like the Dix–Hallpike, fails to identify the cause or to distinguish peripheral vs. central or unilateral vs. bilateral deficits. Testing is not medically necessary for typical BPPV that is diagnosed clinically and resolves with repositioning maneuvers, repeat testing after symptom resolution is not covered, and VEMP and other vestibular tests not specified in the policy are considered experimental/investigational.
