Covers ambulatory EEG (AEEG) and video EEG (VEEG) monitoring for diagnosis and classification of seizure disorders (including ictal/interictal recording), evaluation of intractable epilepsy and surgical candidacy, neonatal HIE on therapeutic hypothermia, and related indications, while digital analysis of EEG (DEEG) is considered not medically necessary. Major limitations/requirements: the policy is inactive and not for current claims adjudication; AEEG is typically diagnostic within 24–72 hours (extended monitoring beyond ~72 hours or for unattended/non‑cooperative patients is generally not covered), and VEEG is limited to specified indications and must be interpreted in the clinical context.
