This policy addresses navigated transcranial magnetic stimulation (nTMS) as a noninvasive preoperative functional mapping tool to localize eloquent motor and language cortex in patients with brain lesions (e.g., tumors) undergoing surgical planning to reduce risk of motor or language deficits. Major limitations/requirements: the policy is inactive and considers nTMS experimental/investigational and not covered; intraoperative direct cortical stimulation (DCS) is usually required for confirmation, accuracy depends on patient task performance, and spatial concordance with DCS/fMRI/MEG is variable.
