This policy covers allergy management services including skin testing (percutaneous, intradermal, patch/photo-patch), serum specific and total IgE testing, inhalational challenge, medically supervised oral food challenges, and allergen immunotherapy for patients with suspected or confirmed allergic disease (e.g., IgE‑mediated inhalant, food or venom allergies, asthma/airway hyperresponsiveness, ABPA, selected immunodeficiencies, and contact dermatitis). Coverage requires clinically appropriate selection of tests (serum IgE generally only when skin testing is contraindicated or not feasible), excludes many provocation/alternative tests as experimental or unproven (e.g., nasal/conjunctival challenges, IgG/cytotoxic tests, provocation‑neutralization), and is subject to member benefit, state regulations, and verification; the policy is currently inactive and not for claims adjudication.
