This policy addresses the Antigen Leukocyte Antibody Test (ALCAT), a laboratory-developed blood assay that measures leukocyte size/number changes to identify suspected food and environmental intolerances. The test is considered experimental, investigational, and/or unproven and not covered for any indication—including non–IgE-mediated food intolerance, gastrointestinal, dermatologic, or systemic symptoms— and it lacks FDA clearance, robust comparative/clinical utility evidence, has variable panels and coding, and is subject to CLIA LDT requirements and the member’s benefit plan.
