This policy covers salivary testing—including late‑night salivary cortisol for evaluation of Cushing’s syndrome, salivary hormone assays, and (per Illinois statute) home saliva cancer screening for asymptomatic high‑risk patients every 24 months or when symptomatic at a physical exam—when ordered by a physician or other qualified health professional. Major limitations: most other salivary uses (e.g., saliva‑based cancer diagnosis, menopause monitoring, CKD screening, periodontal disease, ovulation prediction) are considered experimental/unproven and not covered; coverage is determined by the member’s benefit plan and applicable state rules, and the Illinois mandate applies only to specified lines of business and effective policy dates.
