Thyroid function tests (including serum TSH and complementary thyroid hormone measurements) are covered to diagnose, distinguish, and monitor primary and secondary thyroid disorders, to follow patients with thyroid disease or endocrine neoplasms, and when a variety of metabolic, psychiatric, cardiac, ophthalmologic, reproductive, integumentary, neuromuscular, nutritional, or systemic symptoms suggest thyroid dysfunction. Routine testing is typically limited to two tests per year for clinically stable patients, with more frequent testing allowed when therapy is changed or symptoms/signs warrant additional monitoring; appropriate laboratory results and clinical rationale must be documented.
