Policy Summary
Medicare covers general or monitored anesthesia for transvenous cardiac pacemaker implantation only when adequate documentation of medical necessity is provided and reviewed by the Medicare Administrative Contractor (which may consult medical specialists). For thoracic (open) pacemaker implantation, general anesthesia is always used and covered without the need for special documentation.
