Policy Summary
Medicare covers sterilization procedures only when they are necessary parts of treatment for an illness or injury (for example, hysterectomy for tumor or bilateral oophorectomy/orchidectomy for cancer). Elective procedures performed primarily to achieve sterilization, sterilizations intended solely to prevent pregnancy for general health or mental-condition concerns, and sterilizations of intellectually disabled persons performed only to prevent conception are not covered; claims lacking pathological evidence of medical necessity will be denied.
