Medicare nationally covers electrical stimulation (ES) and electromagnetic therapy as adjunctive treatments for chronic (≥30 days, not healed) Stage III/IV pressure ulcers, arterial ulcers, diabetic ulcers, and venous stasis ulcers only after at least 30 days of appropriate standard wound care with no measurable improvement. Coverage requires supervision and performance by a physician, physical therapist, or incident-to-physician service, with baseline and monthly wound evaluations and discontinuation when the wound is 100% epithelized; unsupervised or initial-use therapy and continued therapy without measurable improvement within any 30-day period are not covered.
