Policy Summary
Insulin syringes are covered only when furnished incident to a physician's professional services under §1861(s)(2)(A), administered by the physician or under the physician's direct personal supervision, and given in an emergency (for example, diabetic coma). Self-administration by a diabetic, non-emergency injections, or syringes furnished outside the 'incident to' physician context are not covered.
