Policy Summary
Medicare does not cover vitamin B12 injections to strengthen tendons, ligaments, or related structures of the foot. Coverage is denied because there is no evidence of effectiveness for that purpose and the injections are a nonsurgical treatment excluded under the subluxation exclusion, making them not reasonable and necessary under §1862(a)(1).
