Medicare covers granulocyte transfusions for patients with severe infection when granulocytopenia is present, typically defined as fewer than 500 granulocytes/mm3. Covered situations explicitly include granulocytopenia with evidence of gram-negative sepsis and granulocytopenia in febrile patients with progressive localized infections thought to be gram-negative that have not responded to appropriate antibiotics; documentation of labs and clinical course should support the claim.
