Policy Summary
Scalp hypothermia (ice-filled bags, bandages, or cold-chemical devices) to prevent chemotherapy-induced hair loss may be provided as a supply included with chemotherapy treatment, but Medicare will not recognize or separately reimburse a charge for these supplies. The policy contains no additional documentation, prior authorization, or frequency limits.
