Medicare covers post-implant follow-up and evaluation services for implanted cardiac pacemakers with these guidelines focused on lithium battery-powered units; mercury-zinc units remain governed by earlier (1980) guidance until replaced. The treating physician is responsible for determining individualized monitoring frequency (with prescriptions required and renewed at least annually when monitoring is performed by third parties), dual-chamber devices programmed to ventricular-only pacing are treated as single-chamber for screening, and MACs must include both clinic and transtelephonic monitoring data when evaluating claim frequency.
