Policy Summary
FOBT is covered to evaluate known or suspected GI bleeding, unexpected or iron deficiency anemia possibly related to GI blood loss, and abnormal signs or patient complaints such as black or red-tinged stools. Testing is reported once to cover up to three stool specimens; in NSAID users with a history of GI bleeding but no other symptoms, testing is generally limited to once every three months. When FOBT is used for asymptomatic colorectal cancer screening, it should be billed with the colorectal cancer screening HCPCS code for fecal-occult blood test (1-3 simultaneous determinations).
