Medicare covers CT scans (head and body) when they are reasonable and necessary for the individual patient, supported by medical/scientific evidence, and performed on CT equipment models known to the FDA and in full market release. MPDI is covered as part of a covered CT, and mobile CT scans are covered under the same medical necessity and equipment-approval rules provided state radiation-control laws are followed and appropriate physician supervision and documentation (including hospital privileges or an attending order) are maintained. No national coverage determination exists for CTA; coverage decisions for CTA are deferred to local MACs, and claims may be denied for unsupported indications, excessive frequency, noncompliant equipment, or missing documentation.
