Medicare covers EKG services, including computer analysis, IDTF-performed EKGs with physician interpretation, transtelephonic transmissions, home EKGs with documented medical necessity, Holter monitoring (~24 hours), event monitors (typically up to 30 days), and implantable recorders when clinically indicated. Coverage requires documented clinical indications (not screening except the Welcome to Medicare exam), adherence to equipment and staffing standards for transtelephonic and 24-hour attended monitoring, and local MAC discretion for FDA-approved devices that must be categorized per the policy framework.
