Medicare covers a defined regimen of routine laboratory tests for dialysis (CRD) patients including per-dialysis hematocrit/hemoglobin and clotting tests, weekly prothrombin time for patients on anticoagulants and serum creatinine, monthly BUN, quarterly core chemistries/enzymes and tri-monthly serum aluminum/ferritin. Hepatitis B screening (HBsAg and anti-HBs) is required at dialysis entry and thereafter surveilled according to vaccination and serologic status per the policy table; tests outside routine frequencies or nonroutine tests require documented medical justification and independent laboratory certification when applicable.
