Policy Summary
Challenge ingestion food testing is covered on an outpatient basis when reasonable and necessary for the individual patient and is an accepted diagnostic technique for food allergies. The procedure is not considered reasonable and necessary (and is not covered) when used to diagnose rheumatoid arthritis, depression, or respiratory disorders. Claims should document outpatient status and medical necessity.
