Policy Summary
Medicare covers surgically induced lesions (ablation/destructive procedures) of nerve tracts to control chronic or acute pain from conditions such as terminal cancer and lumbar degenerative arthritis. Covered techniques include rhizolysis, chemical neurodestruction, and radio-frequency lesioning; payment is available in selected cases with concurrence by the Medicare Administrative Contractor's medical staff. These ablation procedures are distinct from implanted-electrode neurostimulation and this policy applies specifically to destructive denervation procedures.
