This Durable Medical Equipment Reference List (NCD190) specifies which DME items are covered, covered with conditions, or denied. Coverage is generally allowed when items are medically necessary for home use and meet specific clinical criteria (e.g., respiratory impairment, mobility criteria in §280.3, post‑operative total knee replacement), require physician prescription or MAC medical-staff approval, or meet documented trial/training requirements; many comfort, convenience, nonreusable disposable, and environmental-control items are explicitly denied under statutory exclusions (e.g., §1861(n)).
