Percutaneous Transluminal Angioplasty (PTA)

Medicare covers PTA for atherosclerotic obstructive lesions in specified peripheral (iliac, femoral, popliteal) and upper-extremity vessels, for select single-vessel coronary disease when CABG is the likely alternative, for renal artery disease after failed medical management, for AV dialysis fistula/graft dysfunction, and for carotid or intracranial procedures only under specified device, imaging, and trial/study conditions. Carotid PTA concurrent with stenting is covered when using FDA-approved carotid stents and embolic protection devices for symptomatic stenosis ≥50% or asymptomatic stenosis ≥70%, with required pre/post neurological assessments, specified imaging confirmation, documented shared decision-making, and institutional carotid-stent program standards; other PTA indications not specifically covered by the NCD are non-covered or are at MAC discretion.
