Policy Summary
Ultrafiltration monitors are covered by Medicare when used as a component of hemodialysis for ESRD patients and when used to calculate fluid removal rates for patients with difficult fluid management, with medical necessity determined case-by-case. Ultrafiltration performed independent of conventional dialysis, and devices designed exclusively for that purpose, are considered experimental and are not covered. Documentation must support medical necessity and that the device is used in the context of hemodialysis care.
