Policy Summary
Medicare does not routinely pay for a specified list of obsolete diagnostic and cardiovascular tests because they have been replaced by more advanced procedures. These tests may only be paid if the performing physician documents satisfactory medical justification in the medical record, and either the QIO (if applicable) or the A/B MAC will determine whether the justification is adequate.
