Medicare covers home health services for blind diabetics who are home confined, under physician care, and require intermittent skilled nursing or physical therapy or speech-language pathology, with a physician-established and periodically reviewed plan documenting a recurring need. Filling insulin syringes alone is not a skilled nursing service and is not separately payable unless the patient otherwise qualifies for home health services or state law prevents aides from prefilling; adequately trained home health aides may prefill syringes for qualified patients (typically on an intermittent/weekly basis).
