Policy Summary
Medicare covers home health nurse visits to teach or administer subcutaneous low-dose heparin for physician-prescribed, homebound patients who are pregnant or who require anticoagulation for DVT, PE, or other conditions when warfarin cannot be tolerated. Nursing administration is reimbursable daily, 7 days/week, for up to 6 months; pregnant patients may continue beyond 6 months without extra documentation, while non-pregnant patients require documented physician justification to extend treatment past 6 months.
