Endothelial cell photography is covered when reasonable and necessary for documented endothelial disease (e.g., cornea guttata, corneal edema), certain corneal dystrophies or syndromes (posterior polymorphous dystrophy, ICE), preoperative assessment for higher-risk intraocular procedures (secondary IOL, cataract surgery, phacoemulsification, some refractive surgery), or for post‑intraocular‑surgery extended-wear contact lens fitting. If performed as part of a presurgical comprehensive or combination brief/intermediate exam for cataract when cataract is the only problem, it is covered only as part of that exam and not additionally billed separately.
