Policy Summary
Self-contained pacemaker monitors (digital or audible/visible) are covered for patients with a cardiac pacemaker when prescribed by a physician; payment may be for rental or purchase. Digital monitors require physician involvement only if the pacemaker rate changes by ≥5 pulses per minute from the initial rate, and audible/visible monitors require involvement when their signals change; if outpatient pacemaker evaluation is performed in addition to home monitoring, medical necessity for that outpatient evaluation must be documented.
