Coverage requires detailed physician documentation of diagnosis/prognosis, objective measurements, prior treatments tried and failed, a treatment plan specifying pressure/frequency/duration, and documented clinical response to initial device use; HCPCS E0652 (segmented calibrated gradient) is covered only in narrowly defined unique circumstances.
