Mechanical/hydraulic incontinence control devices are covered for urinary incontinence caused by permanent anatomic or neurologic bladder dysfunction when reasonable and necessary. Collagen implants are covered for stress urinary incontinence due to intrinsic sphincter deficiency (ISD) when pre-treatment diagnostic criteria (history/physical, cystometrogram, ALLP <100 cm H2O for females or provoked leakage test for males) are met, physicians are appropriately trained, and treatment conforms to limits (initial maximum of five injection sessions; additional sessions require medical justification).
