Policy Summary
For Medicare beneficiaries, ABPM is nationally covered for diagnosing hypertension when there is suspected masked hypertension meeting specified office BP (systolic 120-129 mm Hg or diastolic 75-79 mm Hg on two visits with ≥2 measurements per visit) plus ≥2 out-of-office readings ≥130/80 mm Hg, and for suspected white coat hypertension per the policy definition. Devices must meet technical and procedural requirements, testing must include documented office/out-of-office measurements, and ABPM is covered once per year; other indications are determined by MACs.
