Medicare nationally covers a broad list of ultrasound diagnostic procedures (Category I) across neurologic, ocular, cardiac, thoracic, abdominal, obstetric, vascular and other organ systems when performed for the indicated clinical evaluations, including ultrasound guidance for procedures (e.g., biopsies, aspirations, thoracentesis, pericardiocentesis, amniocentesis). B-scan for atherosclerotic narrowing of peripheral arteries is nationally non-covered (Category II), and other ultrasound uses not listed require local MAC review and possibly medical justification; documentation of medical necessity and MAC claims review is expected.
