Policy Summary
Medicare nationally covers home PT/INR monitoring for chronic oral anticoagulation management in patients on warfarin with mechanical heart valves, chronic atrial fibrillation, or venous thromboembolism (DVT/PE) when prescribed by a treating physician per 42 CFR 410.32(a). Coverage requires at least 3 months of prior anticoagulation, completion of a face-to-face anticoagulation education program with demonstration of device use, continued correct device use, and self-testing no more than once weekly; all other indications are left to local contractor discretion.
