Policy Summary
SPECT is covered for various diagnostic uses including evaluation of stress fractures, spondylosis, spinal infection (e.g., discitis), bone tumors (e.g., osteoid osteoma), assessment of organ blood flow including myocardial viability, and differentiation of ischemic heart disease from dilated cardiomyopathy. Frequency limits and additional documentation requirements are left to the Medicare Administrative Contractor's discretion. For myocardial viability, FDG PET may be performed after an inconclusive SPECT, but SPECT is not permitted following an inconclusive FDG PET.
