NCD300 (FDG PET for Lymphoma) is retired and removed from the NCD Manual; it provides no active coverage criteria. The retired policy was replaced by section 220.6.17 effective April 3, 2009, and that section should be consulted for current FDG PET for lymphoma coverage, documentation, and frequency limits.
