Policy Summary
CMS has determined that lumbar artificial disc replacement (LADR) is nationally non-covered for Medicare beneficiaries over 60 years of age (effective August 14, 2007, and also for Charite discs during May 16, 2006–August 13, 2007). For beneficiaries 60 years of age and younger there is no national coverage determination and coverage is made by local Medicare Administrative Contractors; Medicare IDE trial coverage for non-Charite discs during May 16, 2006–August 13, 2007 was not affected.
