CMS nationally covers implanted permanent single-chamber or dual-chamber pacemakers for documented non-reversible symptomatic bradycardia due to sinus node dysfunction or second- and third-degree AV block. Coverage requires documentation that bradycardia is symptomatic and non-reversible (symptoms attributable to heart rate <60 bpm); multiple asymptomatic conditions and reversible causes (e.g., medication effects, electrolyte disturbance, bradycardia during sleep) are explicitly non-covered. Other indications not specified in this NCD are adjudicated by Medicare Administrative Contractors under section 1862(a)(1)(A).
